We are committed to the notion that our most vulnerable population should have access to the best and most current evidence-based care. From the moment we took on this project, this book has been a labor of love. The entire process has been punctuated with extreme highs and lows. Throughout it all, we enjoyed incredible support from our families, friends, and colleagues. Sadly, in 2013, we lost two of our most faithful and constant supporters: Kimberly Holmes (Linda’s daughter) and Estelle Drozdz (Sandi’s mother). Our book is dedicated to their memory. These two amazing women never met, but both demonstrated remarkable strength in their lives. We are forever in their debt.
