Perhaps the most important aspect of diabetes care is the relationships between the person with diabetes and the members of their diabetes care team. One essential element of this partnership is empowering persons with diabetes through self-management education. Encourage people with diabetes to learn all they can about how to successfully manage their diabetes, including appropriate insulin dose adjustments in response to meals, physical activity, and blood glucose monitoring results. Stress the importance of actively engaging in their diabetes self-management, including glucose monitoring, recordkeeping, healthy eating, and engaging in appropriate levels of physical activity. Healthy living with diabetes is largely driven by an individual’s efforts at diabetes self-management.
