Many ongoing projects in diabetes care delivery or manpower training involve the interface of university-based subspecialty teams in diabetes with primary care workers in outlying communities. The development of functional community contacts is an integral part of diabetes education projects at various universities, including a diabetes education program (the Kentucky Diabetes Program) based at the University of Kentucky. In these and similar diabetes programs, establishing functional and convenient community contacts is often difficult and frustrating.
