Introduction: Continued use of the personal continuous glucose monitoring (CGM) device is recommended in people with diabetes who are treated with intensive insulin therapy during hospitalization to achieve better glycemic control and reduce hypoglycemia. However, initiation of the new real-time CGM (rt-CGM) in hospitalized adult patients has not been approved by the US FDA. This study aims to evaluate the accuracy and feasibility of initiating Glunovo i3 rt-CGM in hospitalized patients with type 2 diabetes mellitus who are admitted to the general medical floors.
Methods: The Glunovo i3™ rt-CGM sensor and transmitter were placed on the patient’s abdomen. The medical personnel calibrated the device with capillary blood glucose (CBG). Paired CBG and sensor glucose values were analyzed for accuracy of CGM using mean absolute relative difference (MARD) and Clarke Error Grid Analysis (CEGA).
Results: Twenty-five patients with type 2 diabetes mellitus were enrolled. Mean age was 72.8 ± 11.6 years and 64% were female. Mean HbA1c was 10±3.1%. All patients required insulin during admission. Mean duration of CGM use was 7.7±2.4 days, and the mean calibration was 2.3±0.7 times/day. There were 397 paired CBG and CGM measurements. The overall MARD was 12.2 ± 12.2%. The lowest MARD was observed in the CBG range of more than 180 mg/dL (11.0 ± 11.1%) and on day 9 of sensor wear (6.8 ± 7.1%). The percentages of glucose readings within ±15%/15 mg/dL, ±20%/20 mg/dL, and ±30%/30 mg/dL of CBG values were 72.8%, 82.9%, and 91.7%, respectively. A total of 96.7% of the data points were in zone A and B of CEGA, and none were in zone E. Percent time in range on day 1 was 41.2 ± 26.2 and improved to 48.3 ± 22.3 in the last 72 hours of sensor wear.
Conclusion: Rt-CGM use in non-critically ill hospitalized insulin-treated type 2 diabetes patients demonstrates high accuracy and potentially improves glycemic control.
