Background: Transition planning should start early and topics covering healthcare navigation and autonomy be discussed, however, whether this is consistently implemented is unclear. At our center, we are working on implementing transition readiness classes to better prepare our youth (age 16-19) for transitioning care. A standardized questionnaire assessing transition readiness (Table 1) was administered prior to attending this class.

Results: n=21. ∼60% of subjects stated that they were unprepared or neither prepared nor unprepared. Figure 1 shows the subject’s perspective of their transition readiness.

Discussion: There are clear deficits in several key aspects of healthcare navigation and autonomy. These findings support the need for a structured transition program which should include periodic assessments of transition readiness.
