Visual Abstract

The Control-IQ technology Observational (CLIO) study prospectively follows a large, diverse cohort with type 1 diabetes with varied insulin use and baseline glycemic control. To study insulin use on the t:slim X2 pump with Control-IQ technology, we compared insulin delivery during the study to standard formulas for predicting correction factor (CF = 1,800/total daily dose) and carbohydrate ratio (CR = 450/total daily dose).

Basal/bolus ratios, CF prediction factor (CF-PF = total daily dose * CF) and CR prediction factor (CR-PF = total daily dose * CR) were analyzed in 1,499 participants who uploaded at least 21 days of pump data to Tandem’s t:connect® web application and had ≥75% CGM use during this time. Data were compared by age group with the Kruskal-Wallis H-test.

Percent basal insulin delivered (including automated increases and decreases in basal delivery) was lower in younger children (40.3%) vs. adults (45-53%, p<0.001). CF-PF was significantly greater in children and teenagers, with the highest CF-PF of 2,380 in the 14-18-year age group, then gradually decreasing to 1,647. CR-PF was lowest in children age 6-9 and in adults age ≥65, at 360 and 387, respectively (Table 1).

Insulin use with the t:slim X2 pump with Control-IQ technology differed from standard prediction formulas across age groups in this study cohort. Examining insulin use patterns when adjusting pump settings may assist with optimizing outcomes.
