Object: This study aims to investigate the relationship between sudomotor function and coronary atherosclerosis in patients with type 2 diabetes.

Methods: A total of 147 patients with type 2 diabetes were enrolled (64.60% male, 59.58±10.20 years). Using sudoscan detector to assess the mean electrochemical skin conductance (ESC). Patients were divided into three groups according to the mean hands and feet ESC: sudomotor severe dysfunction group (SDF: ESC < 40μS), sudomotor moderate dysfunction group (MDF: 40μS ≤ ESC ≤60μS) and normal sudomotor function group (NSF: ESC > 60μS). The severity of coronary atherosclerosis was determined according to Gensini score.

Results: Gensini score was significantly different among the three groups, with an increasing trend from NSF to MDF and SDF[3.50(0.00, 12.00) vs. 6.50(2.13, 12.25) vs. 10.50(4.86, 22.75), P=0.010]. Spearman correlation analysis revealed a negative correlation between the Gensini score and hand ESC (r=-0.2, P=0.013). After adjusting for confounding factors, multiple linear regression analysis indicated that ESC was significantly associated with coronary atherosclerosis (OR=0.100, 95% CI: 0.012-0.875, P=0.038).

Conclusions: In patients with type 2 diabetes, the severity of coronary atherosclerosis increased with impaired sudomotor function, and sudomotor dysfunction is independently associated with coronary atherosclerosis.
