Models include the following: age (5-year category); time period (10 periods); BMI (<21, 21–22.9, 23–24.9, 25–28.9, 29–31.9, and ≥32 kg/m2); cigarette smoking (never, past, and current smoking of 1–14, 15–24, and ≥25 cigarettes/day); menopausal status (premenopausal, postmenopausal without hormone replacement, postmenopausal with past hormone replacement, and postmenopausal with current hormone replacement); and parental history of MI before 60 years of age.
